| Stage 1 | Increase ≥0.3 mg/dL, or ≥26.4 µmol/L, or increase to 1.5–2.0 × baseline | <0.5 mL/kg/hour for >6 hours |
| Stage 2 | Increase to >2.0–3.0 × baseline | <0.5 mL/kg/hour for >12 hours |
| Stage 3 | Increase to >3.0 × baseline, or serum creatinine ≥4.0 mg/dL with acute increase ≥0.5 mg/dL, or renal replacement therapy | <0.3 mL/kg/hour for 24 hours, or anuria for 12 hours |
