ACC/AHA Heart Failure Staging Calculator

This tool classifies patients into ACC/AHA heart failure stages A–D and provides stage-based therapy considerations. It is for education and clinical decision support only, not a substitute for clinician judgment.

1. Risk factors for heart failure Hypertension Diabetes mellitus Coronary artery disease, prior MI, or peripheral vascular disease Obesity or metabolic syndrome Exposure to cardiotoxic drugs, chemotherapy, radiation, or alcohol misuse Family history of cardiomyopathy

2. Evidence of pre-heart failure / structural disease Reduced left ventricular ejection fraction or systolic dysfunction Left ventricular hypertrophy Significant valvular heart disease Prior myocardial infarction Elevated filling pressures, diastolic dysfunction, or abnormal cardiac imaging Elevated BNP/NT-proBNP or persistently elevated cardiac troponin without symptoms

3. Symptoms or signs of heart failure Dyspnea, orthopnea, or paroxysmal nocturnal dyspnea Edema, fluid retention, pulmonary congestion, or elevated JVP Fatigue or reduced exercise tolerance due to suspected HF Previous or current clinical diagnosis of symptomatic heart failure

4. Advanced heart failure features Recurrent HF hospitalizations or urgent visits despite guideline-directed medical therapy Marked symptoms at rest or with minimal activity despite optimized therapy Need for continuous IV inotropes or temporary mechanical circulatory support Being evaluated for transplant, durable LVAD, or advanced HF therapies End-organ dysfunction, cachexia, or severe functional limitation related to HF

5. Optional ejection fraction category Unknown / not applicable HFrEF: LVEF ≤ 40% HFmrEF: LVEF 41–49% HFpEF: LVEF ≥ 50% HFimpEF: prior LVEF ≤ 40%, now improved to > 40%

Why this stage was selected

This tool does not diagnose heart failure, estimate prognosis, or replace clinician judgment. Treatment should be individualized based on blood pressure, kidney function, potassium level, rhythm, comorbidities, contraindications, patient goals, and local protocols.
