Age >60 years
LDH > normal

ECOG Performance Status >1
Ann Arbor stage III or IV
More than one extranodal disease site

May include disease in bone marrow, CNS, liver/GI tract, or lungs.

Kidney and/or adrenal gland involvement

Score 0–1
Approximate 2-year CNS relapse risk: 0.6%

Intermediate Risk

Score 2–3
Approximate 2-year CNS relapse risk: 3.4%

High Risk

Score 4–6
Approximate 2-year CNS relapse risk: 10.2%
