Brief Addiction Monitor (BAM)

A 30-day, categorical-response assessment of substance use, relapse risk factors, and recovery-supportive behaviors.

Clinical use: BAM supports measurement-based care and progress monitoring. It is not a stand-alone diagnostic or emergency assessment.

1In the past 30 days, how would you say your physical health has been?

2In the past 30 days, how many nights did you have trouble falling asleep or staying asleep?

3In the past 30 days, how many days have you felt depressed, anxious, angry, or very upset throughout most of the day?

Alcohol and drug use

Items 4, 5, and 6 contribute to the Use score. Item 5 is shown only when alcohol use is reported.

4In the past 30 days, how many days did you drink any alcohol?

6In the past 30 days, how many days did you use any illegal/street drugs or abuse any prescription medications?

Recovery supports and environment

Items 8, 11, and 15 contribute to Risk; items 9, 10, 12, 13, 14, and 16 contribute to Protection.

8In the past 30 days, how much were you bothered by cravings or urges to drink alcohol or use drugs?

9How confident are you in your ability to be completely abstinent from alcohol and drugs in the next 30 days?

10In the past 30 days, how many days did you attend self-help meetings, such as AA or NA, to support your recovery?

11In the past 30 days, how many days were you in situations or with people that might put you at an increased risk for using alcohol or drugs? For example, around risky “people, places, or things.”

13In the past 30 days, how many days did you spend much of the time at work, school, or doing volunteer work?

14Do you have enough income from legal sources to pay for necessities such as housing, transportation, food, and clothing for yourself and your dependents?

16In the past 30 days, how many days were you in contact or spent time with family members or friends who are supportive of your recovery?
