The Mini-Mental State Examination (MMSE)

The Mini-Mental State examination (MMSE) is a commonly used set of questions for screening cognitive function. This examination is not suitable for making a diagnosis but can be used to indicate the presence of cognitive impairment, such as in a person with suspected dementia or following a head injury.

1. Orientation to Time (0–5)

Date, day, month, year, and season. Award 1 point for each correct response.

2. Orientation to Place (0–5)

State, country, city, building, and floor. Award 1 point for each correct response.

3. Registration (0–3)

Name three unrelated objects and ask the patient to repeat them. Award 1 point for each correctly repeated word.

4. Attention and Calculation (0–5)

Serial sevens from 100, or spelling “WORLD” backwards. Award up to 5 points.

5. Recall (0–3)

Ask the patient to recall the three words given earlier. Award 1 point for each correctly recalled word.

6. Language: Naming (0–2)

Ask the patient to name two common objects. Award 1 point for each correct answer.

7. Repetition (0–1)

Ask the patient to repeat the designated phrase exactly. Award 1 point if correct.

8. Three-Stage Command (0–3)

Follow the three-part paper command. Award 1 point for each correctly completed step.

9. Reading (0–1)

Follow the written instruction. Award 1 point if the action is performed correctly.

10. Writing (0–1)

Write a meaningful sentence containing a subject and a verb. Award 1 point if correct.

11. Copying (0–1)

Copy the intersecting pentagons accurately. Award 1 point when both shapes and their intersection are correct.
