ABC Score Calculator for Massive Transfusion

Assessment of Blood Consumption score for adult trauma patients.

Clinical use note: This calculator is an educational aid only. It does not replace clinical judgment or your local trauma / massive transfusion protocol. In unstable trauma patients, do not delay resuscitation or MTP activation while completing a score.
1. Penetrating mechanism of injury?
2. Positive FAST?
3. Systolic blood pressure < 90 mmHg?
4. Heart rate > 120 beats/min?

Scoring

  • Penetrating mechanism of injury: 1 point
  • Positive FAST: 1 point
  • Systolic BP < 90 mmHg: 1 point
  • Heart rate > 120 beats/min: 1 point

Total score: 0–4 points.

Common interpretation: ABC score ≥2 suggests higher likelihood of requiring massive transfusion / MTP activation, depending on institutional policy and clinical context.

Some references use SBP ≤90 mmHg and HR ≥120 bpm wording. Follow your institution’s preferred cutoff definitions.
