Important: this calculator implements the original 2019 PCP-HF model, which was developed for adults aged 30–80 years without prevalent cardiovascular disease and uses only the published groups White male, White female, Black male, and Black female. Newer AHA PREVENT equations are a different tool.

The PCP-HF Risk Score stands for Pooled Cohort Equations to Prevent Heart Failure. It is a clinical prediction tool that estimates a person’s 10-year risk of developing heart failure before they have symptoms, mainly for primary prevention in adults without known cardiovascular disease.

The original model was developed for adults roughly 30 to 80 years old and uses routine clinical data such as age, systolic blood pressure, blood pressure treatment, smoking, diabetes/glucose treatment, total cholesterol, HDL cholesterol, body mass index, and QRS duration, with sex- and race-specific equations in the original publication.

In plain language, it is meant to answer: “How likely is this otherwise asymptomatic adult to develop heart failure over the next 10 years?” That estimate can help guide prevention discussions about blood pressure, weight, smoking, diabetes control, and other risk-reduction steps.

One important detail: PCP-HF is not the same as the newer AHA PREVENT calculator. PREVENT is a newer family of equations that estimates broader cardiovascular risk, including heart failure, whereas PCP-HF is the older heart-failure-specific tool.
