The Comprehensive Difficult IV Access Scoring Tool (C-DIVA) is a bedside screening score used to identify patients who are at risk of difficult peripheral IV access before repeated failed attempts happen. Its purpose is to help clinicians recognize likely difficult access early and escalate to a more experienced inserter or vascular access specialist when appropriate.

C-DIVA uses 4 categories, each scored 0 to 2, for a total score of 0 to 8:
visible appearance of the veins, palpable sites, history of difficult access, and clinical factors. Higher scores mean a higher likelihood of difficult IV access.

The commonly reported interpretation is:

  • 0–3 = low risk
  • 4–5 = medium risk
  • 6–8 = high risk
    These bands are meant to guide next steps, such as routine insertion for low-risk patients and earlier escalation for higher-risk patients.
