The Eosinophilic Esophagitis Endoscopic Reference Score (EREFS) is a standardized scoring system used during upper endoscopy to describe how severe the visible esophageal changes are in eosinophilic esophagitis (EoE).
It scores 5 endoscopic findings:
- Edema
- Rings
- Exudates
- Furrows
- Stricture
Typical scoring is:
- Edema: 0–1
- Rings: 0–3
- Exudates: 0–2
- Furrows: 0–1
- Stricture: 0–1
That gives a total score from 0 to 8.
How to read it:
- Lower score = fewer visible endoscopic abnormalities
- Higher score = more severe endoscopic disease
Why it is used:
- to standardize endoscopy reports
- to measure disease severity
- to track response to treatment over time
- to compare findings across visits or studies
It is an endoscopic severity tool, not a diagnosis by itself. Diagnosis of EoE also depends on the clinical picture and biopsy findings.
