Patient Assessment

Applies to: Patients >16 years with blunt head trauma

Exclusions: Neurological deficits, bleeding disorders/anticoagulants, pregnancy, open skull fractures, seizures, or unstable trauma patients

1. Glasgow Coma Scale (GCS) score:
Is the GCS score < 15 at 2 hours after injury?

2. Suspected open or depressed skull fracture:
Are there any clinical signs of open or depressed skull fracture?

3. Any sign of basal skull fracture:
Evidence of basal skull fracture (e.g., raccoon eyes, Battle’s sign, hemotympanum, CSF otorrhea/rhinorrhea)?

4. Vomiting episodes:
Has the patient vomited ≥ 2 times?

5. Age factor:
Is the patient ≥ 65 years old?

6. Amnesia before impact:
Is there ≥ 30 minutes of amnesia of events before the impact?

7. Dangerous mechanism:
Was the injury caused by a dangerous mechanism (e.g., pedestrian struck by vehicle, occupant ejected from vehicle, fall from height >3 feet or 5 stairs)?

About the Canadian CT Head Rule

The Canadian CT Head Rule was developed to identify patients at high risk of requiring neurosurgical intervention and clinically important brain injury. It has been validated in patients > 16 years of age, with blunt head trauma.

A comparison between the New Orleans Criteria and the Canadian CT Head Rule was performed in 2005. It showed both had 100% sensitivities, but the Canadian CT Head Rule was more specific (50.6% vs 12.7%) and thus could result in fewer CT Scans.
