Calculates the Acute Physiology and Chronic Health Evaluation II score using the most abnormal values recorded during the first 24 hours of ICU admission.

Patient Details

Qualifying conditions include severe organ insufficiency or immunocompromise present before admission.

Acute Physiology Variables

Enter the worst value for each variable during the first 24 hours.

Use PaO₂ when FiO₂ is below 0.50.
Use arterial pH when an arterial blood gas is available.
Creatinine points are doubled when acute renal failure is present.
GCS contribution is calculated as 15 minus the actual GCS.
Clinical note: APACHE II is a severity-of-illness score and should not be used alone to direct treatment decisions or estimate individual mortality without clinical context and admission diagnosis.

APACHE II provides predicted mortality percentages based on the total score:

  • Score 0-10: Predicted Mortality ~ 4%
  • Score 11-20: Predicted Mortality ~ 18%
  • Score 21-30: Predicted Mortality ~ 35%
  • Score 31-40: Predicted Mortality ~ 57%
  • Score >40: Predicted Mortality ~ 84%
