The Mini-Mental State Examination (MMSE)

The Mini-Mental State examination (MMSE) is a commonly used set of questions for screening cognitive function. This examination is not suitable for making a diagnosis but can be used to indicate the presence of cognitive impairment, such as in a person with suspected dementia or following a head injury.

Ask the patient: What is the date? Day? Month? Year? Season?
1 point for each correct answer.
Ask: Where are we? (state, country, city, building, floor)
1 point for each correct response.
Name 3 unrelated objects clearly and slowly. Ask the patient to repeat all three.
1 point for each correctly repeated word.
Ask patient to count backward from 100 by sevens (i.e., 100, 93, 86…)
1 point for each correct subtraction (max 5), or spell “WORLD” backwards.
Ask the patient to recall the 3 words mentioned earlier.
1 point for each correctly recalled word.
Show two common objects (e.g., pencil and watch). Ask the patient to name them.
1 point for each correct name.
Ask the patient to repeat: “No ifs, ands, or buts.”
1 point if repeated exactly.
Give a paper and say: “Take the paper in your right hand, fold it in half, and put it on the floor.”
1 point per correct step.
Show a written instruction “Close your eyes.” and ask the patient to do it.
1 point if action is performed correctly.
Ask the patient to write a sentence. It must have a subject and verb and make sense.
1 point if correct.
Show a pair of intersecting pentagons. Patient must copy them accurately.
1 point if both shapes are correctly drawn and intersect.
