The HACOR score is a bedside tool used to predict failure of non-invasive ventilation (NIV) in patients with acute hypoxemic respiratory failure. It helps clinicians decide whether NIV is likely to succeed or whether early intubation should be considered to avoid delayed treatment and worse outcomes.

How to Interpret
HACOR is usually assessed 1–2 hours after starting NIV in acute hypoxemic respiratory failure. Higher scores indicate a higher risk that NIV will fail and intubation will be required.
| HACOR score | Approximate NIV failure risk* | Risk band |
|---|---|---|
| 0–5 | ≈ < 20% | Low |
| 6–10 | ≈ 20–40% | Intermediate |
| 11–15 | ≈ 40–70% | High |
| > 15 | ≈ > 70% | Very high |
*Ranges based on published cohorts with original and updated HACOR scoring. Actual risk varies with diagnosis, comorbidities, and local practice. Always combine with full clinical assessment and guidelines.
