SOFA-2 is a 2025 update of the original SOFA score for critically ill adults. It keeps the same 6 organ systems—brain, respiratory, cardiovascular, liver, renal, and hemostasis—with each scored 0 to 4, for a total score of 0 to 24. The update added contemporary support variables and revised thresholds, including delirium treatment in the brain domain, noninvasive respiratory support and ECMO in the respiratory domain, updated vasopressor thresholds in the cardiovascular domain, renal replacement therapy criteria in the renal domain, and a revised platelet cutoff in the hemostasis domain.

One caution: the published SOFA-2 papers describe it as a tool to describe organ dysfunction consistently, not primarily as a mortality calculator. So the interpretation below is a practical severity summary rather than an official validated mortality table.
