About the Seattle Heart Failure Model
The Seattle Heart Failure Model (SHFM) is a validated prognostic tool designed specifically for patients with heart failure. It estimates survival probability and life expectancy based on clinical, laboratory, and therapeutic factors.
Purpose and Use
The SHFM helps clinicians and patients understand the likely course of heart failure, guiding treatment decisions and discussions about advanced therapies like heart transplantation or ventricular assist devices.
Key Factors Considered
- Demographics: Age, gender
- Clinical Status: NYHA Class, ejection fraction, blood pressure, weight
- Laboratory Values: Sodium, hemoglobin, lymphocyte percentage, uric acid, cholesterol
- Medications: ACE inhibitors/ARBs, beta-blockers, MRAs, statins, allopurinol
- Devices: ICDs, CRT devices
Unique Feature
The SHFM is unique because it not only predicts natural history but also quantifies how specific evidence-based medications and devices can improve a patient’s prognosis.
Limitations
The model may be less accurate for patients with very recent acute decompensations, those on newer heart failure medications not included in the original model, or those with significant non-cardiac comorbidities.
