The Revised Intensity Battle Score (RIBS) is a clinical risk score for adult trauma patients with rib fractures. It was developed as an updated version of the earlier Battle score to better predict which admitted patients are more likely to have a poor hospital outcome.

RIBS is used to estimate the chance of complications such as any complication, ICU upgrade or unplanned intubation, prolonged ventilation, pneumonia/VAP, and death after rib-fracture injury. External validation studies have evaluated it against those outcomes in hospitalized trauma patients.

The revised score is based on factors including:

  • number of ribs fractured
  • chest tube placement
  • pulmonary contusions
  • COPD
  • Glasgow Coma Score (GCS)
    Age was removed in the revised version because it was not predictive enough in the development study.

In simple terms, a higher RIBS score means a higher likelihood of a poor outcome after rib fractures, so it can help with triage and level-of-care decisions.
