1. High-risk surgery

(e.g., intraperitoneal, intrathoracic, suprainguinal vascular)

2. History of ischemic heart disease

(Hx of MI, angina, Q-waves on ECG, positive stress test, or current nitroglycerin use)

3. History of congestive heart failure

(Hx of CHF, pulmonary edema, S3 gallop, or JVD)

4. History of cerebrovascular disease

(Hx of stroke or TIA)

5. Preoperative treatment with insulin

6. Preoperative serum creatinine > 2.0 mg/dL

(or > 177 µmol/L)
