The PRECISE-DAPT Score is a clinical risk score used after PCI (coronary stenting) to estimate a patient’s out-of-hospital bleeding risk while on dual antiplatelet therapy (DAPT). It helps clinicians decide whether a shorter or longer DAPT duration is more appropriate by balancing bleeding risk against ischemic benefit.

It is calculated from 5 variables:

  • Age
  • Hemoglobin
  • White blood cell count
  • Creatinine clearance
  • History of prior spontaneous bleeding

In routine use, a score of 25 or higher is generally considered high bleeding risk. Patients in that range are often considered for shorter DAPT, while lower scores suggest relatively lower bleeding risk.

A simple way to think about it:

  • Lower score = bleeding risk is lower
  • Higher score = bleeding risk is higher, so prolonged DAPT may be less favorable

It is a decision-support tool, not a stand-alone rule. Doctors still interpret it together with things like the reason for PCI, stent complexity, prior heart attack risk, kidney function, anemia, and the patient’s overall situation.
