The Pediatric Fibrosis Score-Continuous (pFIB-c) is a noninvasive risk score designed to help estimate the likelihood of significant liver fibrosis in children and adolescents with metabolic dysfunction–associated steatotic liver disease (MASLD). It was developed mainly as a rule-out tool to help identify pediatric patients who are unlikely to have clinically important fibrosis.
It uses routine clinical data, including sex, hypertension, ALT, weight Z-score, HOMA-IR, and Black ethnicity, and converts them into a continuous probability score. Higher values mean a higher estimated probability of fibrosis.
In the published study, pFIB-c was developed to exclude significant fibrosis, defined as stage F2 or higher, and a cutoff around 0.226 was used as the main threshold to support rule-out of significant fibrosis.
So in simple terms, pFIB-c is a pediatric MASLD fibrosis screening calculator that helps decide which children may be lower risk and which may need further assessment, such as elastography or specialist evaluation. It supports clinical judgment rather than replacing it.
