Troponin testing and full ACS workup still depend on symptoms, timing, ECG findings,
clinician judgment, and local protocol.
The scoring choices in the calculator follow the standard HEART component table: History 0–2, ECG 0–2, Age 0–2, and Risk factors 0–2, where the highest risk-factor category is 3 or more risk factors or known atherosclerotic disease.
The most important interpretation threshold is HEAR ≤1, which multiple studies use as the very-low-risk group when asking whether troponin testing can be omitted in selected patients. That said, the decision to skip troponin is not universal and should follow local ED/chest-pain pathway rules.
