Please select the most appropriate description for the patient:

People who are robust, active, energetic, and motivated. These people commonly exercise regularly. They are among the fittest for their age.
People who have no active disease symptoms but are less fit than category 1. They may exercise occasionally or regularly, but not as intensely.
People whose medical problems are well controlled, but symptoms limit their activity somewhat (e.g., they walk more slowly than others their age, or need help with heavy housework).
While not overtly dependent, these people often complain of being “slowed up” and often require help with shopping, finances, heavy housework, or stairs. They are often less active than others their age.
These people often have limited daily activity outside of their home due to symptoms. They commonly need help with light housework, medication management, and may have difficulty with stairs.
People who need help with all outside activities and with light housework. They often have difficulty with personal care (e.g., bathing, dressing) and may need supervision with some tasks.
Completely dependent for personal care from severe symptoms or severely disabled. They may have advanced disease and are usually bedridden or restricted to a chair.
Completely dependent, approaching end of life. Typically bedridden, requiring full care and often palliative care.
Approaching end of life, usually within 6 months, and not otherwise obviously frail. This score applies to people with a life-limiting illness whose frailty is not their main determinant of prognosis.
