ARC-HBR Criteria Calculator

Identifies patients undergoing percutaneous coronary intervention who are at high risk of major bleeding or intracranial hemorrhage.

Major Criteria

Anticipated use of long-term oral anticoagulation Severe or end-stage chronic kidney disease (eGFR < 30 mL/min/1.73 m²) Hemoglobin < 11 g/dL Spontaneous bleeding requiring hospitalization or transfusion in the past 6 months, or recurrent at any time Baseline thrombocytopenia (platelet count < 100 × 10⁹/L) Chronic bleeding diathesis Liver cirrhosis with portal hypertension Active malignancy within 12 months (excluding nonmelanoma skin cancer) Previous spontaneous intracranial hemorrhage at any time Previous traumatic intracranial hemorrhage within the past 12 months Moderate or severe ischemic stroke within the past 6 months Nondeferrable major surgery while on dual antiplatelet therapy Major surgery or major trauma within 30 days before PCI

Minor Criteria

Age ≥ 75 years Moderate chronic kidney disease (eGFR 30–59 mL/min/1.73 m²) Mild anemia: hemoglobin 11–12.9 g/dL (men) or 11–11.9 g/dL (women) Spontaneous bleeding requiring hospitalization or transfusion within the past 12 months (but not meeting major criterion) Long-term oral NSAID or steroid use Any ischemic stroke at any time not meeting major criterion
