Menopause Rating Scale (MRS): what it is and how it’s used
The Menopause Rating Scale (MRS) is a validated, self-completed questionnaire designed to measure the severity of age-/menopause-related symptoms and how much they affect a woman’s health-related quality of life. It’s widely used in both clinical practice (to structure symptom reviews and monitor change) and research (to compare symptom burden across groups and over time). Springer
What the scale contains
The MRS includes 11 symptoms, each rated by the woman from 0 to 4:
- 0 = no complaints
- 1 = mild
- 2 = moderate
- 3 = severe
- 4 = very severe Springer+1
The 11 items are: hot flushes/sweating, heart discomfort, sleep problems, depressive mood, irritability, anxiety, physical & mental exhaustion, sexual problems, bladder problems, vaginal dryness, and joint/muscle discomfort. Hormone Balance
Subscales (domains)
Scores can be summarized into three domains (subscales), plus a total score:
- Somatic/vegetative (4 items): hot flushes, heart discomfort, sleep problems, joint/muscle discomfort (items 1–3 & 11)
- Psychological (4 items): depressive mood, irritability, anxiety, exhaustion (items 4–7)
- Urogenital (3 items): sexual problems, bladder problems, vaginal dryness (items 8–10) Erken Menopoz+1
The total MRS score is the sum of all items (range 0–44). Springer
Interpreting the total score (common categories)

- 0–4: no/little complaints
- 5–8: mild complaints
- 9–15: moderate complaints
- 16–44: severe complaints
